All parents of students entering a District school for the first time shall present an original, certified birth certificate. Unless registered previously, all kindergarten and first grade students must present an original, certified birth certificate. Students should be registered by their parents. To be eligible to enter school, kindergarten students must be five years old on or before September 1, and grade one students must be six years old by the same date.

The State of Illinois requires a Physical Examination of all pupils in public elementary schools immediately prior to or upon their entrance into prekindergarten or kindergarten or grade one and grade five, and all new residents who cannot furnish evidence of a physical examination in the previous school attended. The eligibility of children to enroll in these grades is contingent upon the completion of the Physical Examination. The completed form should be submitted to the school on the day of registration.

Proof Of Residency

Parent or Guardian of all students attending District 152 schools shall be required to furnish two (2) proofs of residency at the beginning of each school year or whenever a parent or guardian desires to transfer a student into the District. Proof of residency certification shall be signed by the parent or guardian. For proof of residency, the District will accept only the following items:

  • Current (IL) State ID (with current address where you reside)

  • Valid (IL) Drivers License (with current address where you reside)

  • Home Mortgage/Closing Statement or Real Estate Tax Bill

  • One Current Utility Bill: Gas, Electric, Land Line Phone, Cable, Water (Cell Phone bills are not accepted)

  • Voters Registration Card

All of the items listed above must reflect the address located within the district and shall be certified by the parent or guardian. No student will be permitted to attend school in this district until the proof residency requirements have been satisfied.

If any of the above information is missing, you must complete an affidavit. All Kindergarten, 6th Graders, and New Students must have a current (within 12 months) Illinois Physical. All Kindergarten Students must present proof of a complete eye examination.

New Students/Transfer Students

  • IL State Board of Education (ISBE) Transfer Form

  • Out of State School Transfer Form

  • Current IEP (if required)

  • Complete 906 Placement Form with Caseworker's Name and Agency Information required

  • Kindergarten and First Grade Students must meet the September 1 entry deadline

  • Original Birth Certificate

  • Current Illinois Physical

  • Immunization Record

Textbook Rental and Fees

All fees include a non-refundable Technology Fee of $10.00

Grades K-8: $30.00*

*Grades 7 & 8 require an additional fee of $15.00 for gym uniform
